STUDENT RESPONSIBILITY
I hereby am making application to Legacy Bible and Counseling Institute and I understand that the registration fee will be retained to cover the cost of processing my application.

I hereby certify that this application is true and complete to the best of my knowledge. Upon acceptance, I agree to comply properly with the rules and regulations of the institution.

FINANCIAL AGREEMENT
• I understand that a registration fee of $150.00 is due before I can receive my textbooks or begin classes with for credit. This registration payment is not part of my tuition payments.

• I understand that my tuition may be paid in full, or by making nine (9) monthly payments starting on September 1st, continuing on the first of each month through May 1st.

• I understand that matriculation fees for graduation are not included in my monthly payment, unless special arrangements have been made with LBCI administration to include them in my payment plan. This will increase the amount of my monthly payments.

• I understand that until my balance is paid in full I may be able to participate in graduation; however, no credits will be available for transfer for unpaid classes, and no degrees shall be delivered until balance is paid in full.

By signing below, I testify that I have read and understand the above financial policies and do hereby agree to fulfill my financial obligations as so stated.
